Teen Access to Medical Records Policy Statement

After Hours Pediatrics recognizes that teens have special privacy rights concerning their medical history and records. To ensure this privacy, the records of patients 14 years of age or older will not be visible unless the patient has formally requested portal access.

Teen patients may choose to make their records visible under the parent’s portal

account (as they are today), or they may choose to have their own portal account with a separate login. Medical records will only be visible on the portal if the patient makes one of these choices.

To make these records visible on the portal, teen patients must sign the Teen Portal Access Form. This form is available on our website, and it is also available at any of our four locations. All requests require the signature of the patient. Parents cannot make this request on behalf of their children.

We realize that for most families there is no issue with having teen medical records visible on the parent’s portal account. However, we need to protect the privacy of some of our teen patients.
